ENROLLMENT REQUIREMENTS CHECKLIST
All information must be complete for enrollments. Please check off each document that is in the file.
Proof of Custody or Guardianship – Picture ID must be provided. If not the natural parent, a letter of legal guardianship is required for registration. Parent can contact Gwinnett County Probate Court to obtain guardianship information (770-822-8000)

Proof of Residency
_____ A copy of settlement statement or lease agreement (check dates)
_____ Proof of service for gas, electricity or water (current utility bill)
_____ Notarized Residency Affidavit (if applicable)
Health Requirements
_____ Immunization Record (GA Dept. of Human Resources #3231)
_____ Vision/Hearing/Dental Record (GA Dept. of Human Resources #3300)
_____ 30 Day Extension Form (only if coming from out of state)
Withdrawal Paperwork
_____ Withdrawal form from previous school
_____ Transfer/Withdrawal grades
_____ Transcript (for rising 9th graders, need 8th grade report card showing promotion)
_____ IEP (if student is in special education)
_____ Gifted Eligibility records (if student is in gifted program)
_____ ESOL Eligibility records (if student is in ESOL program)
Students new to the county and current “out of state” ESOL students need to report
to INC before enrolling
Discipline – Discipline statement from previous school is required.
